What is Cataract Surgery?

A cataract is a condition where the natural lens of the eye becomes cloudy, impairing vision. Cataract surgery involves:

1. Removing the cloudy lens.
2. Replacing it with a clear intraocular lens (IOL) to restore clear vision.

This procedure is minimally invasive, usually takes less than 30 minutes, and has a short recovery period.

Common Causes of Cataracts

• Aging – The most common cause, as the lens naturally becomes less transparent over time.
• Diabetes, which increases the risk of early cataracts.
• Prolonged exposure to ultraviolet (UV) light.
• Eye injuries or previous eye surgeries.
• Long-term use of corticosteroids.
• Smoking and excessive alcohol consumption.
• Genetic factors – A family history of cataracts may increase the risk.

Diagnosis and Medical Examinations

To diagnose cataracts and determine the need for surgery, an eye doctor performs the following tests:

1. Visual Acuity Test – Measures how clearly you can see.
2. Slit-Lamp Examination – Examines the lens for cloudiness.
3. Tonometry Test – Measures eye pressure to rule out glaucoma.
4. Pupil Dilation Test – Allows a detailed view of the lens and retina.
5. Biometry Test – Measures the eye to determine the correct power for the replacement lens.

Types of Cataract Surgery

1. Phacoemulsification (Phaco - Ultrasound Cataract Surgery)

• The most commonly used technique.
• A small incision is made in the cornea, and ultrasound waves break the cloudy lens into tiny fragments.
• The fragments are removed, and an artificial intraocular lens (IOL) is implanted.
• No stitches are required, leading to a faster recovery.

2. Extracapsular Cataract Extraction (ECCE - Traditional Surgery)

• Used for advanced cataracts that are too dense for phacoemulsification.
• The lens is removed in one piece through a larger incision.
• An intraocular lens is inserted, and stitches are required.
• Longer recovery time compared to phaco surgery.

3. Femtosecond Laser-Assisted Cataract Surgery (FLACS)

• Uses laser technology to create precise incisions and soften the lens.
• Offers greater precision and faster recovery, but costs more.

How the Surgery is Performed

1. Preparing for Surgery:

• Pre-surgery eye exams to determine the correct lens power.
• Stopping certain medications, such as blood thinners, if required.
• Using dilating eye drops before the procedure.

2. Steps of the Procedure:

1. Local anesthesia (eye drops or injection) is administered to numb the eye (patient remains awake but feels no pain).
2. A tiny incision is made in the cornea.
3. Ultrasound or laser energy is used to break up and remove the cloudy lens.
4. A clear intraocular lens (IOL) is implanted in place of the natural lens.
5. The incision self-heals in most cases without stitches.

3. After Surgery:

• The patient can go home the same day.
• Eye drops are prescribed to prevent infection and reduce inflammation.
• Blurry vision may persist for a few days but gradually improves.

Recovery After Cataract Surgery

1. First Week After Surgery:

• Avoid rubbing or touching the eye.
• Wear an eye shield at night to prevent accidental injury.
• Avoid strenuous activities, bending over, or lifting heavy objects.
• No swimming or exposure to unclean water for at least two weeks.

2. 2-4 Weeks Post-Surgery:

• Vision continues to sharpen gradually.
• A new prescription for glasses may be needed.
• Most patients can resume normal activities after a few weeks.

Possible Complications of Cataract Surgery

Although the surgery is very safe, rare complications may include:

• Infection or inflammation (Endophthalmitis).
• Increased eye pressure (Temporary Glaucoma).
• Retinal detachment (Rare but serious).
• Posterior Capsule Opacification (PCO) – Cloudiness after surgery, which can be treated with a simple laser procedure.
• Temporary dry eye or light sensitivity.

Success Rate and Long-Term Outcomes

• Cataract surgery has a success rate of over 98%, making it one of the safest and most effective surgeries in the world.
• Most patients regain sharp, clear vision within days to weeks.
• Some may need reading glasses post-surgery, especially if a standard lens is used.
