Congenital Heart Surgery

Congenital heart surgery is a highly intricate surgical discipline dedicated to correcting the anatomical abnormalities of the heart and major blood vessels that occur during fetal development.

These defects, collectively known as congenital heart disease (CHD), present in numerous forms, affecting how blood flows through the heart and to the rest of the body. The complexity of congenital heart surgery stems from the wide spectrum of possible defects and the need to operate on patients ranging from fragile newborns to adults with complex, sometimes previously repaired, conditions.

Surgical approaches vary widely and are determined by the specific defect, its severity, and the patient's age. Procedures can include repairing holes in the heart walls (septal defects), reconstructing or replacing heart valves, widening narrowed blood vessels (such as the aorta or pulmonary artery), and rerouting blood flow in complex conditions like transposition of the great arteries or hypoplastic left heart syndrome.

Many procedures require the use of a heart-lung bypass machine to maintain circulation while the heart is stopped for repair.

 Significant advancements in surgical techniques, technology, and pre and post-operative care have dramatically improved outcomes for individuals with CHD, allowing many to lead active lives, although some may require staged procedures or lifelong follow-up.

The symptoms of congenital heart defects requiring surgery vary greatly depending on the type and severity of the defect. Some defects may cause immediate, severe symptoms in newborns, while others might not present until later childhood or even adulthood.

Common symptoms in infants can include bluish discoloration of the skin, lips, and nails (cyanosis) due to low oxygen levels, rapid breathing or difficulty breathing, poor feeding and inadequate weight gain, and excessive sweating. Older children and adults may experience shortness of breath, fatigue, palpitations, chest pain, dizziness, or swelling in the extremities.

 Heart murmurs, detected during a physical examination, can be a key indicator of a potential defect.

The exact causes of most congenital heart defects are unknown. However, a combination of genetic and environmental factors is believed to play a role.

Risk factors can include a family history of congenital heart disease, certain genetic syndromes (such as Down syndrome), maternal health conditions like diabetes or rubella infection during pregnancy, and the use of certain medications or exposure to certain substances during pregnancy.

The diagnosis of congenital heart defects often begins before birth with fetal echocardiography, an ultrasound of the baby's heart during pregnancy. After birth, if a defect is suspected based on physical examination, symptoms, or a newborn screening test (like pulse oximetry), further diagnostic tests are performed.

These commonly include echocardiography to visualize the heart's structure and function, electrocardiogram (ECG) to assess electrical activity and rhythm, chest X-ray to evaluate heart size and lung condition, and pulse oximetry to measure blood oxygen levels. More detailed imaging like cardiac MRI or CT scans and invasive procedures like cardiac catheterization may be necessary to fully understand the defect's anatomy and severity.

Cardiac catheterization can also be used for certain interventional treatments to repair less complex defects.

 Surgical treatment for congenital heart defects is highly individualized based on the specific anomaly, the child's age, and overall health. Procedures range from relatively simple repairs like closing septal defects to complex reconstructive surgeries requiring the use of a heart-lung machine.

Many complex defects require a series of staged operations over several months or years. Post-operative care involves close monitoring in an intensive care unit and a recovery period.

Long-term follow-up with a cardiologist specializing in congenital heart disease is crucial throughout the patient's life to monitor heart function, detect potential complications, and manage any ongoing issues.
