Colonoscopy

A colonoscopy requires careful bowel preparation to ensure the colon is completely clean for clear visualization of the lining. Preparation typically involves following a clear liquid diet for one or two days before the examination and taking strong laxative medications (bowel prep) the evening before and the morning of the procedure. The colonoscopy is often performed under moderate sedation or deep sedation to help the patient relax and minimize discomfort. The colonoscope is gently inserted through the rectum and slowly advanced through the colon. The physician introduces air or carbon dioxide through the scope to inflate the colon and improve visualization. The camera at the tip of the scope transmits live images to a monitor, allowing the physician to examine the lining carefully as the scope is slowly withdrawn. If polyps or suspicious areas are found, the physician can remove them using small instruments passed through a channel in the scope (polypectomy) or take small tissue samples (biopsy) to be sent to a laboratory for examination. The procedure itself usually takes 30 to 60 minutes.

Colonoscopy is a diagnostic and therapeutic procedure used to evaluate symptoms related to the colon and rectum and detect diseases that may cause them. The procedure itself does not cause disease symptoms (apart from some temporary discomfort) but is performed to help determine the cause of the patient's complaint or to screen for specific conditions. Symptoms or signs that may prompt a physician to order a colonoscopy include:

• Persistent change in bowel habits: Such as ongoing diarrhea or constipation, or a change in the shape or size of the stool.
• Blood in the stool or rectal bleeding: The blood may be bright red or dark, or appear in the result of a fecal occult blood test.
• Persistent abdominal pain or cramping: Especially if severe or not improving.
• Unexplained weight loss.
• Feeling fatigued or tired due to anemia caused by chronic blood loss from the gastrointestinal tract.
• Need for early detection of colorectal cancer: As part of a recommended screening program for individuals in certain age groups or with specific risk factors, even if there are no symptoms.
• Follow-up after a history of colon polyps or treated colorectal cancer.
• Evaluation of suspected inflammatory bowel disease: Such as Crohn's disease or ulcerative colitis.

Underlying causes of these symptoms or risk factors that colonoscopy can help diagnose or evaluate include:

• Colon polyps: Benign growths on the lining of the colon, some of which can develop into cancer over time.
• Colorectal Cancer: A malignant growth that often starts in polyps.
• Inflammatory Bowel Disease (IBD): Such as Crohn's disease and ulcerative colitis, which cause chronic inflammation in the digestive tract.
• Diverticulitis: Inflammation or infection in the small pouches (diverticula) that form in the wall of the colon.
• Colitis: Inflammation of the lining of the colon due to various causes.
• Bleeding from the lower gastrointestinal tract: Identifying the source of bleeding.
• Risk factors for colon cancer: Such as increasing age (especially over 45-50), family history of the disease, personal history of colon polyps or inflammatory bowel disease, certain genetic syndromes, smoking, obesity, and poor diet.

Diagnosis" using colonoscopy is based on the direct visualization of the lining of the colon and rectum using the colonoscope, in addition to taking tissue samples (biopsy) of any abnormal-looking areas. The physician evaluates the color, shape, and texture of the lining, looking for any changes such as redness, swelling, ulcers, polyps, or masses.

"Treatment" in the context of colonoscopy includes two main aspects: diagnosis leading to a treatment plan, and in some cases, immediate treatment during the procedure. Stages after preparation and examination include:

• Performing the examination: The entire colon and rectum are examined.
• Taking biopsies and removing polyps: Samples of any suspicious tissue are taken, and polyps found during the examination are removed using the polypectomy technique.
• Sending samples to the laboratory: Biopsies and removed polyps are sent to a pathologist for examination under a microscope to determine their nature (benign, precancerous, or malignant).
• Obtaining biopsy results: Biopsy results typically take several days to a week or longer.
• Discussing results with the physician: The physician explains the findings of the examination and the biopsy results (if taken) to the patient and what they mean for their condition.
• Developing a treatment plan based on the diagnosis:
  • If results are normal or indicate a benign condition: No specific treatment may be needed other than general recommendations for maintaining digestive health and scheduling the next routine screening (in case of screening colonoscopy).
  • If precancerous polyps are found: They are removed during the colonoscopy, which is an important preventive step to prevent the development of colon cancer. The physician recommends a regular follow-up colonoscopy schedule based on the number, size, and type of polyps removed.
  • If colon cancer or other diseases are diagnosed: A comprehensive treatment plan is developed, which may include surgery, chemotherapy, radiation therapy, or other medications, based on the type, stage, and overall health of the patient.
