Epilepsy Surgery

Epilepsy is a neurological condition characterized by recurrent seizures caused by abnormal electrical activity in the brain. Some patients continue to experience seizures despite appropriate anti-seizure medications and may be evaluated for epilepsy surgery.

The type of procedure depends on the location and characteristics of the seizures. Surgical options may include removing the identified seizure focus, disconnecting specific brain pathways, or using neuromodulation techniques to reduce seizure activity.

Before surgery, a specialized epilepsy team performs detailed testing to identify where seizures begin and assess important brain functions such as movement, speech, memory, and vision.

Common Symptoms:

• Loss of consciousness or awareness

• Convulsions or involuntary movements

• Staring spells or periods of unresponsiveness

• Repetitive or unusual movements

• Unusual sensations, smells, emotions, or visual experiences before a seizure

• Confusion or excessive sleepiness after a seizure

• Temporary memory problems

• Sudden falls or loss of muscle control

Possible Causes & Risk Factors:

• Genetic or inherited factors

• Previous brain injuries

• Stroke or other brain damage

• Brain tumors or structural abnormalities

• Central nervous system infections

• Developmental brain disorders

• Brain scars or other structural changes

• In some cases, the exact cause may remain unknown

Diagnosis begins with a detailed medical history, seizure description, and neurological examination. Depending on the case, testing may include EEG, brain MRI, prolonged video-EEG monitoring, neuropsychological assessment, and specialized functional brain mapping.

Treatment usually starts with anti-seizure medications, which may be adjusted or combined according to the patient's response. When seizures continue despite appropriate medication, the patient may be referred to a specialized epilepsy center for a comprehensive surgical evaluation.

If a specific seizure focus is identified and can be safely treated, resective surgery may be considered to remove the affected brain tissue. When resection is not suitable, other approaches such as disconnection procedures or neuromodulation may be considered.

The main goals of epilepsy surgery are to reduce seizure frequency and severity, improve seizure control, minimize the impact of epilepsy on daily life, and support greater independence and quality of life.

Recovery and outcomes vary depending on the type of epilepsy, seizure location, surgical procedure, and the patient's overall health. Each patient requires an individualized assessment by a multidisciplinary epilepsy team.
