What Is Esophagectomy?

An esophagectomy is surgery to remove part or all of the esophagus affected by cancer. The surgeon then reconstructs the food passage—most commonly by using the stomach to create a new tube and connecting it to the remaining esophagus, usually in the chest or neck.

Depending on the tumor location, the procedure may also include removal of a small upper portion of the stomach and nearby lymph nodes.

When Is Esophagectomy Recommended?

  • Esophageal cancer that appears localized or locally advanced but still removable.

  • Cancer at the lower esophagus or gastroesophageal junction.

  • A tumor that has responded to chemotherapy and radiotherapy before surgery.

  • No evidence of distant spread after complete staging.

  • Sufficient heart, lung, liver, kidney, and nutritional status to undergo major surgery.

Not every patient with esophageal cancer requires surgery. If the cancer has spread to distant organs, or if surgery is unsafe because of general health, the oncology team may recommend chemotherapy, radiotherapy, immunotherapy, or supportive treatment instead.

Why Are Chemotherapy and Radiotherapy Sometimes Given Before Surgery?

For many localized but non-early esophageal cancers, treatment is started with chemotherapy and radiotherapy before surgery. This is called neoadjuvant treatment.

It may help to:

  • Shrink the tumor before surgery.

  • Treat microscopic cancer cells that cannot be seen on scans.

  • Improve the chance of removing the tumor completely.

  • Reduce the risk of cancer returning after surgery.

  • Assess how responsive the cancer is to treatment.

The decision is made by a multidisciplinary tumor board after reviewing the biopsy, imaging, and the patient’s general condition.

Preoperative Assessment and Staging

Before deciding on surgery, patients usually require:

  • Upper endoscopy and biopsy to confirm the tumor type.

  • CT scan of the chest, abdomen, and pelvis.

  • PET-CT scan to rule out distant metastasis.

  • Endoscopic ultrasound (EUS) to assess tumor depth and nearby lymph nodes.

  • Blood tests and nutritional assessment.

  • Cardiac and respiratory assessment to ensure fitness for major surgery.

  • Review by oncology, surgery, anesthesia, radiology, pathology, and nutrition specialists.
