Cardiac Intensive Care Unit (CICU / CCU)

A Cardiac Intensive Care Unit (CICU), also known as a Coronary Care Unit (CCU), is a specialized intensive care unit designed for patients with severe, unstable, or life-threatening heart and circulatory conditions who require continuous cardiac monitoring and rapid medical intervention.

Patients admitted to a CICU may have acute coronary syndromes, severe heart failure, dangerous heart rhythm abnormalities, cardiogenic shock, complications following a heart attack, or other cardiovascular conditions that require a higher level of monitoring and treatment than can be provided in a standard hospital ward.

The unit combines intensive care capabilities with specialized cardiovascular expertise. Patients can undergo continuous electrocardiographic monitoring, blood pressure and oxygen monitoring, advanced hemodynamic assessment, laboratory testing, cardiac imaging, respiratory support, and other forms of organ support when required.

Treatment focuses on stabilizing the patient's heart and circulation while identifying and treating the underlying cardiovascular problem. Depending on the condition, this may involve medications, electrical cardioversion or defibrillation, temporary cardiac pacing, mechanical ventilation, circulatory support, coronary angiography, percutaneous coronary intervention (PCI), or other emergency cardiac procedures.

CICU care is typically delivered by a multidisciplinary team that may include cardiologists, critical care physicians, interventional cardiologists, cardiac electrophysiologists, cardiac surgeons, anesthesiologists, specialized cardiac nurses, respiratory therapists, and other specialists.

Modern cardiac intensive care also extends beyond coronary artery disease. Patients with complex heart failure, severe valvular disease, pulmonary vascular emergencies, post-cardiac arrest conditions, and complications following cardiovascular procedures may require CICU management.

Once cardiovascular function is sufficiently stable and continuous intensive monitoring or advanced organ support is no longer required, the patient may be transferred to a cardiac step-down unit, standard cardiac ward, rehabilitation program, or another appropriate level of care.

Not every patient experiencing chest pain, palpitations, or shortness of breath requires cardiac intensive care. The appropriate level of care is determined through urgent medical evaluation, cardiovascular assessment, and evaluation of the patient's overall clinical stability.

Cardiac intensive care may be required when a cardiovascular disease or complication causes severe instability of the heart or circulation.

Conditions that may require CICU or CCU admission include:

• Acute myocardial infarction (heart attack)

• Acute coronary syndrome

• Cardiogenic shock

• Acute or severe heart failure

• Life-threatening cardiac arrhythmias

• Ventricular tachycardia or ventricular fibrillation

• Severe bradycardia or heart block

• Cardiac arrest and post-cardiac arrest care

• Unstable angina with high-risk features

• Acute myocarditis

• Severe cardiomyopathy

• Acute complications of valvular heart disease

• Aortic emergencies

• Pulmonary embolism with cardiovascular instability

• Severe pulmonary hypertension with right-heart failure

• Complications following coronary angiography or PCI

• Complications following cardiac surgery or other cardiovascular procedures

• Mechanical complications following myocardial infarction

Some high-risk patients may also require temporary admission to a cardiac intensive care unit for advanced monitoring following complex cardiovascular procedures.

Assessment in a Cardiac Intensive Care Unit aims to identify the underlying cardiovascular problem, determine its severity, evaluate the stability of the circulation, and detect complications as early as possible.

Diagnostic evaluation may include:

• Continuous electrocardiographic (ECG) monitoring

• 12-lead ECG

• Continuous blood pressure monitoring

• Oxygen saturation monitoring

• Cardiac biomarkers, including troponin when indicated

• Complete blood count

• Kidney and liver function tests

More advanced investigations may be required depending on the suspected cardiovascular condition.

Assessment continues throughout the patient's CICU stay. Heart rhythm, blood pressure, circulation, respiratory status, organ function, laboratory results, and response to treatment are continuously or repeatedly evaluated so that therapy can be adjusted as the patient's condition changes.

Treatment in a Cardiac Intensive Care Unit depends on the underlying cardiovascular condition and the degree of instability affecting the patient's heart, circulation, breathing, and other organ systems.

Acute Coronary Syndrome and Heart Attack Treatment

Patients with acute coronary syndrome or myocardial infarction may require medications to reduce clot formation and cardiac workload, continuous ECG monitoring, pain management, and urgent restoration of coronary blood flow when appropriate.

Some patients require coronary angiography followed by percutaneous coronary intervention (PCI), including balloon angioplasty and coronary stent placement.

Treatment of Cardiac Arrhythmias

Dangerous heart rhythm abnormalities require rapid assessment and treatment.

Depending on the arrhythmia and the patient's stability, treatment may include antiarrhythmic medications, electrical cardioversion, defibrillation, temporary cardiac pacing, or other electrophysiological interventions.

Heart Failure Management

Patients with severe or acute heart failure may require oxygen or ventilatory support, intravenous diuretics, medications to improve circulation or cardiac function, careful fluid management, and continuous cardiovascular monitoring.

Cardiogenic Shock

Cardiogenic shock occurs when the heart cannot pump enough blood to maintain adequate circulation to vital organs.

Treatment may involve intravenous medications to support blood pressure and cardiac output, mechanical ventilation, emergency coronary intervention when indicated, and advanced mechanical circulatory support in selected patients.

Respiratory and Organ Support

Severe cardiac conditions can affect the lungs, kidneys, brain, and other organs.

Patients may require oxygen therapy, non-invasive respiratory support, mechanical ventilation, renal replacement therapy, or other forms of organ support while the underlying cardiovascular condition is treated.

Advanced Cardiac Support

Selected critically ill cardiac patients may require advanced technologies such as temporary mechanical circulatory support.

The specific device or intervention depends on the underlying condition, severity of cardiovascular failure, available expertise, and clinical assessment.

Recovery and Cardiac Rehabilitation

Once the patient's cardiovascular condition stabilizes and intensive monitoring is no longer required, they may be transferred to a cardiac step-down unit or standard hospital ward.

Following the acute phase, some patients may benefit from cardiac rehabilitation, medication optimization, nutritional and lifestyle management, physical activity planning, and long-term cardiology follow-up.

The overall objective of CICU treatment is to stabilize the heart and circulation, rapidly treat the underlying cardiovascular emergency, prevent further organ damage and complications, and safely transition the patient toward recovery and longer-term cardiovascular care.
